Consider the possibility that the report is exactly what it says.
Whatever is in slot 4 is not an amanda tape, is what used to be
tape "day-04" but has been overwritten, is what used to be tape
"day-04" but is now defective, or ???

Can you read part of it with tools like dd and determine what
data is on the tape?
