That is a tape error, not a Bacula error.

Step outside bacula for a moment and use HP's Library and Tape tools or 
other diagnostics to solve errors BEFORE trying to continue.

What user is bacula-sd running as?

What are the ownership and permissions on /dev/nst0

What does "mt -f /dev/nst0 status" give you?

etc.
