The entry in known_hosts normally happens the first time a user
connects to a new remote host with ssh and is mostly unrelated to the
keys that permit passwordless login (but of course you have to have
write permission).  The target host key is checked on each subsequent
connection and the client won't connect if the host key doesn't match
- so if you re-install the OS, etc. later, you have to delete the key
from known_hosts and accept it again.
