I've seen that error a few times on my VXA-320. A few weeks after I had that 
problem 
the drive went even worse and would not pass its built-in self test, so 
Tandberg 
replaced it.  Although I haven't seen the error on the new drive, that doesn't 
necessarily mean that the error was definitely a drive problem.

One thing that I have come to believe in when I have problems like this is to 
use the 
software that comes with the drive to erase the tape, then write an EOF on it 
with mt 
and label it in bacula.  On linux, this is vxaTool.  A support tech from 
Tandberg 
once told me that using vxaTool to erase a tape can fix a lot of problems.  One 
thing 
that I have noticed on my system is that a fresh tape will not start reporting 
the 
number of times it has been loaded (./vxaTool /dev/sg4 -i) unless it has been 
erased 
once by the drive.  So now I always erase new tapes once before putting them in 
the 
bacula rotation.

For a tape that is already labeled by bacula, you might want to delete the tape 
from 
the catalog, then erase it as noted above, then create it again in bacula with 
"label 
slots=5 barcodes".  That is because I don't think there is a "relabel" command 
that 
reads barcodes.

Certainly you should try the drive self test (it will overwrite any data on 
your test 
tape), and make sure you have the latest firmware in the drive and the changer. 
 Note 
that older changer firmware did not include the drive test, so you may need to 
update 
that first.
