Wow, 7 slots again. Do you delete a last line? :) Just kidding.
 
The two commands were issued directly one after another because I
worried about the first responses of the changer (RequestSense (0x03)
failed ...).


Please, hard reset your changer (off/on), check all connections, check all firmware options which could disable one slot - cleaning slot, etc. Then check library status with mtx again. It should work. If not, check you HBA firmware and OS driver.
