We also have a autochanger and sometimes we have this problem, too.

Tapes will be marked as error when
* fd crashes and network error or broken datastream occures
* sd crashes
* sd is beeing restared while tape is in drive (sometimes)
* dir is beeing restared

In any cases some jobs are executed, so there is data lose on incomplete jobs 
which is the reason for the wrong filecount.

The only bug i could possibly see is, that the filecount will be also wrong if 
something on the network or with the fd happpens. This should the bacula-dir 
catch, mark the job as error and correct the filecounter on the tape.
