I do use amanda on Irix, but unfortunately I don't have a changer, so I
can't help a lot.  Just an idea, have a look in /var/adm/SYSLOG (which
is Irix' default syslog location) whether there are any messages.  This
should rule out bad memory or cabling issues normally.  (Yes, Irix does
report bad memory, it even tells you which bit is faulty.)
