Okay, I will give this a go. In the meantime I have opened up the smtp
ports on one of my linux boxes which use sendmail to post it's own
notifications and it works fine. I would still rather go straight to my
exchange though :)

Try running bsmtp on the command line again with the -d 10 option and
post the result. That might print useful ids to help track where the
message has gone.
You can run it with a pipe to avoid typing the message and CTRL-D:
echo test | bsmtp ...
