I suggest running the btape tests, to see if they detect anything.
 
It sounds like a problem with the number of EOFs at the end of the tape.  Do
you have 'TWO EOF = yes' in the config?  Maybe the drive is now running with 1
EOF for some reason?  Check with mt geteotmodel.
