     I've seen this before:

     1. when the library (3494) inventory and ADSM got out of sync.  which
     is solved by doing a re-inventory of the library followed by an AUDIT
     LIBR in adsm. (which in my case takes long enough to get dinner in)

     2. Vols were being marked readonly.

     3. Physical carts had write protect on.

Are you using a library?  Have you checked the library to see if there are
scratch volumes available (q libv).
