ANR8944E-Hardware or media error, TSM 5.4 on Windows

Hi,

we have a TSM 5.4.1.2 running on Windows Server 2003, SP2.
We have attached a DELL PV132T tape library ,Fibre Channel, with 2 LTO3 drives (same as a ADIC 24)
HBA is a QLogic QLE 2360.

Before the library was attached to a TSM 5.3.0.0 with the same HBA and was running smoothly.

The changer is configured with the TSM device driver, tape drives are configured with the uptodate IBM tape driver, as recommended.

Config within TSM is attached.

The library is configured and online.
But when trying to checkin tapes or audit the library I get these errors in actlog:

Strange is that some volumes have been labeled.
The tapes in the library have been used without problems until last week.

When I first configured the library with the device wizard the elment number of the 2nd drive was not recognized, I had to enter it manually.
I tried to change element numbers for the drives - same problems...

Help would be really appreciated!

I would check the media physically to see any damage, also check the leader pin if you are running LTO tapes. Leader pin known to pop out. This may give you some headached with the error.

Check the driver, paths and ensure the path are defined correctly
 
KEY=03 is usually a media error. Is it always MT0, or other drives too? Is it always trying the same tape first? If you have other drives, try taking MT0 offline and repeating the operation. The (possibly) bad tape should hit another drive and either: cause the same error, or work fine.

I recognized that these errors also appear on the second drive.
I took a look at the tapes and was quite surprised: Some tapes were quite dirty even the barcode.
I removed all tapes and inserted 5 new ones - TSM labeled all of them without errors.
Tapes have been in use for 2 years now so I ordered some new.
When I get those I will give feedback here.

The strange thing is that our "old" TSM 5.3 was writing to these tapes without problems...
