Hi James,
Bacula is trying to load the Volume "000061" and it should be in slot 4. Bacula gets the slot information for a volume from the catalog. You can check if this volume is in your tape library with a mtx -f /dev/yourchanger status command. And also you could run an "update slots scan" bconsole command to update the catalog with the actual information about volumes and tapes in your tape library.
