If there does not appear to be damage with the tape, I would look into
this error.  People have these a few times a month on this list. I
have not seen this on my side so I have no experience dealing with
this.

I believe the drive/changer was still busy when bacula was feeding it
commands. I have seen this before. I get a similar error if I try to
update slots before the changer is done with inventory.

Yes. Also after bacula hits errors like this I have seen that manual
mounts are required most of the time or even worse rebooting the
changer and restarting of the bacula-sd daemon.
