Ooh.. that's not too good..

If this is linux, tried installing smartmontools and running smartctl -a
/dev/hda? ... also check your drive cable is secure and is the correct
type (80 vs. 40 conductor for ATA-100+ etc.)

This should tell you if the drive is suffering any incorrectable errors
etc. internally.
