A few stories come to mind..
1. Systemstate restore failure is very common (not only with TSM). I
recommend (scheduled) ntbackup for systemstate as an extra failsafe.
2. The Wintel support group excluding lots of stuff from the backup because
there were so many Warning messages in the log...
      Not sure how you can test this...
3. Restore failed because Pri stg pool volume became unavailable after
backup, and thus no copy
4. Restore failed because the customer could not remember the file name,
only that it was about a year ago and it was an excel file (amongst other
zillions)

1 and 3 I see on a regular basis during DR testing, 3 caused by not managing
the TSM environment close enough. 4 was not a test...
