I have a similar problem that is not human error. Some shared DLT drives on
the SAN go DOWN on one of the Media Servers leaving the tapes in the drives,
but another Media Server (or Master Server) still wants to mount usuable
tapes. 

NBU (version 4.5 Solaris, no maintenance patches, using L700 Tape Library)
proceeds to try mounting tapes into the already occupied DOWN drives, marks
the tape as unmountable, FREEZES the tape, then proceeds to look for another
AVAILABLE or ACTIVE tape and again tried to mount the tape into the occupied
drive. It goes through this until all AVAILABLE or ACTIVE tapes have been
tried, and failed, leaving the library with no more tapes to use - all have
been FROZEN, jobs die with a 96 code. Veritas blames StorEdgeTek who in turn
blames Veritas. StorEdgeTek told me that they know of a few sites with this
problem that was solved with MP1. Veritas said they can't confirm that. 

I am about to install MP3 soon in the hope that the problem goes away. In
the meantime, we have a cronjob script that runs occasionally throughout the
day that unfreezes any tapes that have zero kbytes - the rest of the tapes I
run bpverify on and unfreeze by hand...sometimes I juct go ahead and
unfreeze the ACTIVE tapes when I am sure I can do so. 

We have also tried using vmdareq to keep the Master Server as the scan host,
but it has not helped much.

Thank you all for your advice/assistance. It turns out it was a pure human
error. The person who did the tapes messed up and put some tapes in the
slots which were already occupied (the real tape was in the drive). Hence,
the Netbackup got confused, because it couldn't load the tapes in (since
there were tapes already in the drive). Once, I emptied the silo and
re-inventoried it started working.

 Here is the situation - I am running 4.5 MP2 on Solaris 2.8. Suddenly, my
media server started running out of tapes - I get error 96. After some
investigation I had discovered that all the tapes are frozen. I unfroze the
tapes, expired them and delete them from silo. re-inventory the silo - the
tapes shows up ok. However, as soon as, I start backup it freezes all the
tapes again with the following messages (from my both log):

15:45:26.525 [8780] <2> logconnections: BPDBM CONNECT FROM
169.188.0.113.54263 TO 169.188.0.70.13721
15:45:26.682 [8799] <2> io_set_recvbuf: setting receive network buffer to
