I think you block size is wrong. Mine is specified in KB, so for 128KB block
size, I have:

NSR_DEV_BLOCK_SIZE_LTO_ULTRIUM_2=128

LTO uses variable block size just unset 
NSR_DEV_TAPE_BLOCK_SIZE_LTO_ULTRIUM if set.

and also try  tun CDI of  (not use on edit device -> configuration)
try:

mt -f  /dev/nst0

you should get a bock size of 0 (variable)
