What's your Tru64 Unix version ? Multi pathing is only supported from 
version 5.x and you are using /dev/nrmt?h tape driver name ?!? Oups ....
 
Did you upgraded your Tru64 Unix V4.0x to a Tru64 Unix V5.X and continued
to use /dev/nrmt?h that's being linked to /dev/nrmt?h (during Tru64 5.0x 
install update) ?
 
Have a try to run "hwmgr -show scsi -full" and retrieve your tape 
drive using Bus/target/lun informations and look for active and stale paths.
 
Another good way is to reconfigure your jukebox, replacing :
Jukebox control path from [email protected] <mailto:[email protected]>  to 
/dev/changer/mc? and 
tapes /dev/nrmt?h to /dev/ntape/tape?_d1 to take full advantages of 
multipathing.
 
I feel one of your scsi controller or drive could be 
putting bullshit on the scsi bus. You could also ask HP (aka Compaq) to have a 
look at your binary.errlog. Check also how the file 
/var/cluster/members/{memb}/adm/binary.errlog is growing.

        Hello NetWorker gurus:
        
        On a Tru64 Unix server with Power Edition NetWorker 6.1.1, I noticed
        an error I never saw before when an attempt was made to write to an AIT2
        tape in our Qualstar library's six tape drives.
        
        Does anyone have any suggestions on what the source of this problem 
might
        be?
        
        The error is as follows:
