Same error message here: I/O Error.  if mt doesn't work, neighther
will bacula-sd.

Did you took a look at "dmesg"?  I mean, try to manually write to the
tape (again), then try to provoke the error, and when the error just
shows up, run dmesg and see the output, it should say something that
may be useful.
