Some updates:

i've upgraded to
mdserver2-dir Version: 2.4.2 (26 July 2008) i686-pc-linux-gnu debian 4.0
today.. and took away the following settings:
    Two EOF = Yes
Hardware End of Medium = No
Fast Forward Space File = No
    BSF at EOM = yes
ran btape test, no problems..

Currently running a 1.6Tb backup into 5 volumes.. on the 4th volume now, no
errors yet.. I hope the problem won't reoccur now..

Meanwhile, can anyone advise on what are the settings for? Especially the
"Two EOF = yes" kind of strikes me to be the problem of having 2 blocks
differ error..
