I can have solved it, with
# yum install mt-st
The btape test worked anyway.

I'ts not the permissions then as sd is running as root.
[root@lobacula sbin]# ps -ef | grep bacula
root 12313 1 0 10:11 ?
00:00:00 bacula-sd -c /etc/bacula/bacula-sd.conf
I did activate the sleep part in the mtx-changer which solve part of my
problem, the tape actually unloaded and loaded tape from slot 1. But it
failed because of mt seems to be missing. OS = CentOS 5 I will try and
find a mt rpm.
# Increase the sleep time if you have a slow device
# or remove the sleep and add the following:
wait_for_drive $device
sleep 15
exit $rtn

This is only a problem if you are running the bacula-sd as user bacula
and user bacula is not a member of the group disk.
