Still, from Bacula's point of view, the status might be correct.

Is there anything in the system log regarding SCSI problems?
A zombie process usually indicates this process is either still
blocked by outstanding I/O requests, or can't be finally terminated
because the parent process is still busy. Or something similar... so,
what you describe, sounds more like a general OS, kernel, SCSI driver
or tape hardware problem.

Which is what Bacula usually does, more or less. It handles a tape
error just like and end of media situation.
