Looks like you got an error trying to read the tape label from the tape
your changer thinks came from slot 11.

Your changer also barfed trying to unload the tape to put back into slot 11.
Since it was a fatal error it quit looking, so..

because it couldn't get the next tape (since the changer script died).

Perhaps I missed it since I'm jumping in mid-thread, but are you using real
tapes or vtapes (using the FILE driver)?  If it is real tapes, check your
changer to verify that there is a tape in the drive and your changer can
move it properly (using mtx or whatever tools you have available) and check
your system logs for more descriptive SCSI errors.
