Obviously, review any dsmserv.err file which is ostensibly written to
your TSM server directory in a server failure. Also review your
Windows Event Log for device problems: exception handling should
result in some evidence. Also assure that you're using an
appropriate tape device driver, particularly if the failure occurred
as soon as tape reading was attempted.
