Contact support on this. Without seeing the rest of your activity log we can't
really tell what's wrong. But from what you've said, it sounds like you have
an extra reference within one of your DB tables.

It failed because Access: Offsite and it is Empty. If using DRM do a:
q drm 000254
to see what DRM state it is in. Then process it back through DRM states as you
would a returning tape from offsite. That will clear it to Scratch.

Audit apparantly didn't fix it.
Now when I try to update the volume status I get:

Larry - A frequently reported problem.
Try Audit Volume Fix=Yes, which usually takes care of it.
