This usually means that you have a tape in a drive that ADSM doesn't list with Q
MOUNT (like if the server crashed, or you had an earlier changer failure
message). Also, depending on the level of server you are running, there was a
problem with CHECKIN/CHECKOUT and tapes getting put in the wrong slot. I'd bet
on the tape in the drive, however.
