I would guess that there is another driver loaded against this tape drive.
 The name of the drive reported by the ADSM Utilities Red Cross option
should be something like mt4.0.0.1.  The / you are seeing are plain wrong.

Look in the Control Panel Devices and figure out which driver loaded before
the ADSM SCSI driver and disable it.  You can't make ADSM work without ADSM
SCSI having exclusive access to the drive.
