I have a completely different environment (os and tape
library hardware) but I have also had the same thing
happen a few times. Though I cannot say what caused
this, I do not see this being a scsi cable or
terminator issue as this happened to me both pre and
post hardware upgrade where new drives (as well as
cables) were installed.
