I recommend you to check all SW and FW versions (HBA, Switch, MDR).

Furthermore, read the LEGATO TECHNICAL BULLETIN 373: SCSI or TPRLO Reset
Causes Corruption (Windows).

We had the same problems and adding dedicated HBA's for tape devices solved
the problem. This is a general rule in building a SAN (Windows, UNIX, ...).
