We had a problem similar to this, but may not fit your situation.
We moved our ADSM server from and R30 to an R50. When we wired up the 3590
drives, we accidentally swapped 2 of the SCSI cables. When the server booted
up, rmt2 was rmt3 and vice-versa. When we brought up the ADSM server and it
requested a tape mount, the 3494 would put it in rmt2 but would never see
it, we assume because it was waiting to see that tape in rmt3. When we
corrected the problem by swapping the cables, rmdev-ing the devices, and
"cfgmgr"-ing them back in, all was back to normal.
If your system has been up and running for a long time and no hardware
changes have been made, this may not be the problem

I have a call in to support, but thought maybe someone in the list might
have experienced this and could provide some insight. I have ADSM 3.1.2.1
running on AIX 4.3.1. We use a 3494 with 6 3590 tape drives. Our atldd is
at 3.2.1.0 and Atape is at 4.2.1.0. ADSM requests the mount of a tape.
MountWait is set for 30 minutes. The 3494 never mounts the tape, but ADSM
keeps on waiting and never gives up. I can't find my notes, but I think we
ended up finding a tape in the drive. This happened a couple of weeks ago
when I was restoring some tape volumes the robotics had damaged, and I
bounced ADSM. It happened 3 times over 2 days, but not on the same tape(s).

Saturday morning I discovered a client backup had been running for 7 hours
because it was waiting on a tape mount. I bounced ADSM since I couldn't
cancel anything to free it up and took the drive in question (I found it by
using mtlib to query the drives and found the only one that was busy, yet
had nothing mounted in it per ADSM). This morning I found out that a backup
stgpool that started Saturday afternoon was still running, waiting on a tape
mount. I don't know if it's coincidental or not, but it's waiting on the
same tape as the client backup was Saturday morning.

Does anyone know why the 3494 doesn't mount the tape, or ADSM doesn't use
the MountWait parameter?
