Whenever I've seen this sort of thing, it is ALWAYS related to the hardware in
some aspect. The favorite cause is a faulty terminator, but also fault cables,
bent pins, and even loose connections between port/terminator/cable. After
checking everything carefully, I would begin with the least expensive part and
start replacing.
