[ADSM-L] Damaged Index/Table

I think I have some damage in one of my tables/index.
Doing Move Data on some tapes so they can be reassigned. Once a tape had
been emptied/put in scratch, I ejected it from the library and moved it to
another library.
But, depending on what commands/queries you use, the tape is still in the
old library/system while also being in the new library, already reused.

It does not show up in the old library/Q LIBVOLUME.
Thoughts/ideas on how to diagnose this further and/or resolve it?
This is a Linux server. Just recently upgraded from 6.3.4.200 to 6.3.5.100.
