I have more of a question than an answer.
If both hosts have the same shortname, and only the shortnames are appearing in your Node Down events (hostA is Down), or in your other NetView events for that matter, then how do you tell them apart when they arrive? How do your operators tell them apart now? Do you have more than one separate NetView feeding a single TEC?
If so, then the adapter host will be different for each NetView, and you might be able to use that (hostA from NetViewA) to differentiate them, instead of inventing new hostnames.
If not, be aware that in Node Down/UP and Interface Down/Up there is a wealth of information buried in other varbinds of the trap. Varbind 4 often has the object id of the host, or for an interface, both its IP Address and its object id, as well as it's parent host's object id. And recent versions of NetView have expanded some traps with up to nine variables, just to make TEC integration easier.
Use xnmtrap to modify trapd.conf so that for Node Down (58916865) and Interface Down (58916867) the Log message says " $* " rather than " $3 " and wait for some to come in. Then look at them in trapd.log. You will see a wealth of additional information that you probably didn't know was there.
For 7.14 Appendix A of the NetView Admin Guide was updated to give this current information about NetView internal traps.

Don't know about in the ruleset, but you might use printf in the slot map
dialog. I haven't tried this but was just reading about it yesterday. In
the event configuration Slot Map you would put something like this:
hostname PRINTF ("%s-1",$A)

Hello,
NetView 7.1.3 on AIX 4.3
Can we setup a rule to append any suffix to the host name before forwarding
the event to TEC?
for example if the system name is "xyz" this would be in the event as
"xyz-1"
The reason for that is we have duplicate short names
