Hello Patricia,
You might want to try to upgrade
the lto-4 firmware. When we upgraded one of our systems to use lto-4 tape
drives we had a few problems until we upgraded the firmware to the 89B2 level.
We are now testing the 94D4 level.

I am running :
NetBackup Enterprise 6.5.3 - Windows x64 sp2
I added 2 New IBM LTO4 drives to an existing L1400 tape library
with 8 HP Ultrium LTO3 drives.
The drives are shared between 3 media servers with SSO, I added
the license keys.
I used the IBM Ultrium-TD4 SCSI sequential device driver version
6.2.0.1
Scan shows the drives below.
The backups are running and getting these errors and the tapes get
frozen.

TapeAlert Code: 0x04, Type: Critical, Flag: MEDIA, from drive Drive001 (index
8), Media Id M00098
Error TapeAlert Code: 0x06, Type: Critical,
Flag: WRITE FAILURE, from drive Drive001 (index 8), Media Id M00098
