Keath, I have had this error before. It's amazing how many times one tape
will show up in the error logs when this happens. We take care of this by
first deassigning the tape like you mentioned. But then we also force the tape
to expire and then change it back to our scratch pool again. But following
this process it makes the tape usable again and takes care of the errors.

I noticed a tape that should have been expired that was not. I ran bpexpdate
-deassignempty. I get the following error.
Could not update media list, file read failed
file read failed
I ran bpdbm -consistency and am getting a goodly number of bad image headers.
Could this be causing the problem? During the july 4th weekend I had a full
disk occur on the database drive. I had 200,000 failed job messages in my
inbox by Wednesday when I returned the following week.
