What would cause the JAVA MP3 patch not to work successfully?

Cannot update the NB-Java.tar.Z file.  Updates to NB-Java GUI 
on client machines must be installed directly from the patch.
Installation of pack NB_JAV_45_3 FAILED Wed Apr 30 14:44:55 CDT 2003 Rev. 1.38.

MP1 and MP2 were not a problem.
