This is often bad news. The last time I saw the no entitiy found problem
was with a client that ran out of space on the NBU filesystem. Have you
checked the free space on the NBU filesystem partition? If you have run out of
space, you have likely lost the ability to recover some backups without doing a
full import of the backup media. Search the veritas support site for bad image
header; there is a tech note that describes the problem, and how to clean up
after it (but not to repair the damage).
Again, that client was on 3.4; the message might mean something else on 4.5.
