Paul,
ensure you have enabled logging for

bphdb bpbrm bpbackup bprd bpjobd bpcd bptm
user_ops/dbext/oracle

and check your rman logfiles (should be in user_ops/dbext/oracle) for rman
errors - then you should have a better idea of where to start looking for
resolution.
