Did you also install it on the client?

It needs to be there as well, and you should disable OTM, it really doesn't
add anything on an Exchange or SQL server using the Database Extension.

I installed patch 645 and changed the file list to 
Microsoft Information Store:\First Storage Group\

Now I get failed error stautus 71

Do I need to set something on Exchange server? Do I
disable OTM?
