I have seen this happen when something happens with NetWorker and it
fails to read the volume label when it mounts the tape. Do you see an
entry in the daemon.log that correlates to something along the lines of
"expecting volume X and found volume '-'" ?
When this has happened to me, it has usually been a dirty tape drive, an
nsrmmd crash, and having NetWorker inventory the slot (even the "nsrjb
-II -S <slot>" quick inventory) has been enough to get it to say 'Oh, I
know that volume' ... I've complained to EMC that they should attempt to
mount a tape at least twice in different devices before doing this, but
so far, they don't seem to agree with me.
