Hello
If this errors are repeating on different media ( with different drive), 
then i would look into drivers\connectivety errors either from HBA or 
cables.If you note the trend and find that all errors are from one drive, 
then it may be time to either check the connectively ( rule it out)  and 
or replace the drive that may be going bad.
If's its just one tape then retiring it may be best option. ( make sure 
you are cleaning the drives on regular basis).

Another less likely possibility is that the drive itself is starting to
fail or perhaps just needs cleaning.

If it were me, I'd re-label this tape, before using it again. And I
would 
run the backup to tape again.
