One way to accomplish that would be to  backup
D:\Foldername you want to backup i.e 
D:\SQL
SYSTEM STATE:\
REGISTRY:\
etc

I want the benefits of All, but I only need one folder from the D drive. 
How and where do specify an exclusion?

"ALL" gets more than just the C and D drives.  "ALL" includes the system 
state.  However, you can actually do it either way.  It's whatever way 
makes you more comfortable.
