Are you using fiber or scsi?
Is it direct attached or SAN attached?
If SAN attached check the HBA subscriptions.
We had a similar problem where as we over subscribed out HBA to 3 drives as
a result only RMAN recoveries are not working. When we lowered the HBA
subscription to 2 drives per HBA everything is fine.
