I had this EXACT problem with the same card, but with a infortrend storage box.

I was told that mixing non-raid and raid devices might in some
cases lead to issues like this.

so I swapped out the scsi card with anothe LSI (non RAID) card, and things 
started working
perfectly.

The REALLY annoying thing was I managed to get it to work .. sorta ... under 
windows, but
under Linux was a no-go
