do this log entries mean that we have a drive or media failure (or both)?

is full

so far, so good, but then:

Cannot decode block. Verify the device configuration. Tape positioning by
record is disabled.
Meanwhile, the Linux kernel logs:

After writing these entries, the volume goes "rewinding", then "writing,
moving forward 286 file(s)"
Our server's stinit.def matches the one on Legato's website. Any ideas
what's wrong here?
