What works for us (on Solaris):
mt -f /dev/rmt/0cbn offline

Sometimes we have a similar problem:
- tape is stuck in drive
- NW cannot unload it
- sooner or later NW sets the drive to service mode
What helps then is (on Linux):
mt -f /dev/nst0 unlock
and setting the drive back to normal mode.
