Give a look at max consecutive error value set for that
device from device window. Maybe set to a too low value (<20).
You can also have a bad tape in your lib.
Give your daemon.log a look. I also remember to have seen that
behaviour with cleaning tape deposited in backup tape slot range.
Have tries to load/unload tape manually to check if volume
label can be processed. Maybe drive I/O's heads to be dead,
resultng in I/O error and max consecutive errors to be increased.
I got last week 2 drives with same behavior you described.
