Yes,  I've had this before... cloning calls for some tape from months ago..

It appeared to be some media database type corruption.   The tape being
called for was the same all the time.  Looking at this tape it was decided
it was media we did not need..ie,  it was recyclable.  We ended up deleting
this media/savesets with nsrmm.   The problem then went away.   If the media
be asked for is valid data that you must keep I'm not sure what to do.. for
our case standard media database checks (nsrck -L6 etc) and a media database
scavenge did not help.

I did think when it happened to us that there was a mix up with a clientid..
it was when we added a client that the problem started happened.  Either way
it was definitely a media/file index issue.
