[Networker] Backup failing once a week

anyone seen this error message in the NT4 event viewer;

6/5/03,6:15:21 PM,qntmdlt,Error,None,15,N/A,OIPC4190,The device,
\Device\Tape0, is not ready for access yet.
6/5/03,6:15:21 PM,qntmdlt,Error,None,15,N/A,OIPC4190,The device,
\Device\Tape0, is not ready for access yet.
6/5/03,6:15:21 PM,aic78xx,Error,None,11,N/A,OIPC4190,The driver detected a
controller error on \Device\ScsiPort2.

Message in networker;
 \\.\Tape0 reading: The request could not be performed because of an I/O
device error.
 NetWorker media: (warning) \\.\Tape0 moving: tape_rewind rewind failed:
The device is not ready. (268435477)


Running networker 6.2 with a quantum dlt8000 single tape drive HV.
replace scsi card (Adaptec awa-2944uw and driver) and rebooted the machine
and tape drive.

Has anyone had this problem before?
