I'll take a try at this; because, yes, I've seen a lot of strange things
before.

If you go into the volumes window and scroll to the very top do you see
a tape called "-"   (the volume name is dash) ??

When this happened this us; everytime it went to label a blank tape it
came up with the volume name "-".   It then checked the volume/media database
and there was a volume actually called "-"  .  Truly amazing; how it got
there is beyond me.   Used tapes, of course, labeled fine.

Removing this volume in gui solved the problem.
